Per-Diem LPN

As an LPN with TRU Healthcare, you'll provide skilled nursing care that helps patients remain safe, comfortable, and independent in their own homes.

• Provide skilled nursing services to patients in their homes according to established care plans and physician orders.
• Monitor patient conditions, identify changes in health status, and communicate concerns to the appropriate members of the care team.
• Administer treatments as ordered and within the scope of practice.
• Perform nursing assessments and provide appropriate interventions based on patient needs.
• Educate patients and caregivers on medications, treatments, disease management, safety, and other aspects of their care.
• Accurately and promptly document patient care and communicate with the interdisciplinary team.
• Build meaningful relationships with patients and families while providing compassionate, patient-centered care.

• Current, valid Minnesota LPN license.
• CPR/BLS certification.
• Previous home health experience preferred.
• Strong clinical, communication, and organizational skills.
• Ability to work independently while collaborating effectively with the care team.
• Ability to travel throughout the Barnum area
• Reliable transportation and willingness to travel between patient visits. Mileage reimbursement provided.
• A genuine desire to make a difference in the lives of patients and their families.

This is a per-diem, per-visit position. Earn $45 per visit and $35/hr for meetings and trainings.
