Nurse Practitioner- Center For Better Aging
St. Bernard Hospital
Works in collaboration with the multidiscipline team to assess the physical needs of patients that present to the emergency department. S/he will initiate the medical screening exams (MSE) of patients. Orders, interprets, and records results of clinical and diagnostic tests. Prescribes medications and makes recommendations for other therapeutic forms of treatment. S/he will follow established protocol in a collaborative practice agreement with a physician. Consult with Attending physician for difficult cases. S/he will develop treatment plans and treat patients with acute illnesses/minor injuries that present to the clinic. Perform minor procedures when necessary in emergency situations. Possess a broad depth of knowledge and expertise and manage complex clinical and system issues. Contribute to optimal patient outcome.
Qualifications:
Required Education:
- Master's Degree in Nursing and certification as a Certified Registered Nurse Practitioner (CRNP)
- Licensed Registered Nurse in the state of Illinois
- Valid certification in Basic Life Support (BLS)
Required Experience:
- Minimum of two (2) years of experience in clinic settings
Desirable Experience:
- Experience with the Geriatric population
Duties & Responsibilities:
- Provide comprehensive health assessments and demonstrate a high level of autonomy and expert skill in the diagnosis and treatment of many complex problems.
- Advise and educate patients and family members of health promotion and disease preventative measures.
- Plan and design evidence-based education that benefits at all stages in their career development; ensuring regulatory, accreditation, and organizational requirements are included.
- Obtain detailed medical histories; perform physical examinations; formulate diagnosis and treatment plans; and order diagnostic testing.
- Instruct and educate patients regarding compliance with prescribed therapeutic regimens and health maintenance.
- Diagnose and prescribe treatments both pharmacological and non-pharmacological.
- Maintain accurate records for clinical use and QA activities.
- Assist with data gathering, program development, organization of medical records, and quality control.
- Collaborate with staff physicians and/or Medical Director and other members of the healthcare team.
- Protect patient's rights by maintaining confidentiality of personal information.
