The Nurse RN/LPN provides skilled nursing care to geriatric patients in a long-term care and skilled care facility. Reporting to the Director of Nursing, this role supports day and night shift coverage while delivering safe, compassionate, and coordinated patient care.

Responsibilities

  • Assess geriatric patients and monitor changes in condition.
  • Develop and implement individualized care plans.
  • Administer medications safely and accurately.
  • Monitor and document vital signs.
  • Provide wound care and related treatments.
  • Maintain accurate clinical documentation.
  • Educate patients on care needs and treatment plans.
  • Coordinate care, support infection control, and assist with discharge planning.

Required Qualifications

  • Active Registered Nurse (RN) license.
  • Active Licensed Practical Nurse (LPN) license.
