Part-Time Home Health LVN - Follow-Up Visits

Silver Haven Home Health LLC is seeking a Part-Time Licensed Vocational Nurse (LVN) to provide skilled nursing follow-up visits to home health patients throughout the Dallas area. This position is ideal for an LVN with home health experience who is looking for a flexible, patient-based schedule.

Duties

• Complete skilled nursing follow-up visits in patients' homes.
• Monitor patient condition and report changes to the supervising RN.
• Provide medication management and patient/caregiver education.
• Perform wound care and other ordered skilled nursing treatments within LVN scope.
• Follow established plans of care and physician orders.
• Complete accurate and timely visit documentation.
• Communicate and coordinate with the RN, DON, physicians, and care team.

Requirements

• Current and unrestricted Texas LVN license.
• Home health experience preferred.
• Current CPR/BLS certification.
• Strong clinical assessment and documentation skills.
• Valid driver's license, auto insurance, and reliable transportation.
• Ability to travel throughout the Dallas service area.
• Ability to work independently and complete documentation timely.
