Responsibilities:

• Provide direct patient care including administering medications, wound care, and monitoring vital signs
• Assist physicians and RNs in assessing and documenting patient health status and treatment plans
• Coordinate and communicate patient care needs across interdisciplinary healthcare teams
• Educate patients and families on health management, medications, and discharge instructions
• Maintain accurate and up-to-date electronic health records (EHR) and clinical documentation
• Monitor and report changes in patient condition to supervising medical staff promptly
• Ensure compliance with state, federal, and facility healthcare regulations and safety standards

Requirements:

• Valid California LVN license in good standing
• Current CPR/BLS certification required
• Prior experience in adult healthcare, geriatrics, or a clinical setting preferred
• Proficiency in electronic health records (EHR) and medical documentation
• Strong communication, organizational, and critical thinking skills
• Ability to work collaboratively in a fast-paced, team-oriented environment
