We are seeking compassionate and dependable RNs and LPNs to join our growing team on a PRN basis. This position offers flexible scheduling and the opportunity to provide meaningful, one-on-one care to clients in their homes throughout the Salem area.

Responsibilities
- Conduct in-home nursing visits and client assessments (RN responsibilities based on licensure)
- Monitor client health status and report changes in condition
- Provide skilled nursing services within scope of practice
- Assist with medication management and client education
- Collaborate with caregivers, families, physicians, and the clinical team
- Support development and implementation of individualized care plans
- Complete accurate and timely documentation
- Promote client safety, independence, and quality of life
- Current, unrestricted Oregon RN or LPN license
- Home health, hospice, long-term care, assisted living, or acute care experience preferred
- Strong clinical, communication, and documentation skills
- Current CPR certification
- Valid driver's license, proof of insurance, and reliable transportation
- Ability to work independently and manage a flexible schedule
- Flexible PRN scheduling
