We are seeking a Hospice Licensed Vocational Nurse to join our team! You will be responsible for delivering high quality care to assigned patients.

Responsibilities:

Administering Patient Care following the Plan of Care as established by the Physician and RN Case Manager.

Keep RN Case Manager informed of patient condition after each visit.

Reinforce education to Patient, Family, and Caregiver as established by RN Case Manager.

Participate in interdisciplinary meetings to ensure patient needs are met.

Qualifications:

Active current license

Previous experience in nursing or other medical fields

Ability to build rapport with patients and families

Compassionate and caring demeanor

Strong problem solving and critical thinking skills

Ability to thrive in a fast-paced environment
