This full-time Licensed Vocational Nurse (LVN) position focuses on providing dedicated patient care in a home health setting with a consistent 1:1 nurse-to-patient ratio. The role primarily serves mixed age groups and involves working fixed PM and night (NOC) shifts.

Case : San Juan Capistrano, CA
Hours: Mon-Fri 2p-10:30p
G-Tube dependent patient
Case : Aliso Viejo, CA
Hours: Mon-Sat 3p-11p
G-Tube dependent patient
Case : Ladera Ranch, CA
Hours: Mon-Thurs & Sat 10p-6a
Ventilator dependent patient
Case : Trabuco Canyon, CA
Hours: Mon-Fri 7:30a-3:30p
G-Tube patient
Nurse to attend school w/ patient
Pay Range: $30-$35/hr
Case specific pay
Responsibilities
- Provide direct patient care in a home health environment
- Manage duties specific to the night (NOC) shifts
- Monitor and assess patient conditions throughout assigned shifts
- Maintain accurate and timely documentation of care provided
- Collaborate with healthcare team members to optimize patient outcomes
- Respond to on-call duties as needed
Required Qualifications
- Valid LVN license
- Minimum 1 year of nursing experience
Preferred Qualifications
- Experience with G-Tube care
- Tracheostomy care expertise
- Ventilator management experience
