Responsibilities:
- Conduct comprehensive patient evaluations, including history-taking and physical examinations
- Diagnose and treat acute and chronic medical conditions across diverse patient populations
- Develop and implement individualized treatment plans and care management strategies
- Order and interpret diagnostic tests, labs, and imaging studies
- Prescribe medications and manage ongoing pharmacological therapies
- Collaborate with physicians, specialists, and care teams to ensure continuity of care
- Educate patients and families on health maintenance, disease prevention, and treatment options
- Maintain accurate and thorough clinical documentation in the EMR system
- Active California license as a Physician Assistant (PA-C) or Family Nurse Practitioner (FNP)
- Current DEA certification and prescriptive authority
- Experience in primary care, family medicine, or a related clinical setting preferred
- Strong clinical assessment, diagnostic, and critical thinking skills
- Proficiency with electronic medical records (EMR) systems
- Excellent communication and interpersonal skills with patients and care teams
- Bilingual (English/Spanish) a plus given the local patient population
