Status: Full-Time

Salary: $120,000 $140,000 per year

Sign-On Bonus: $15,000 (Commitment period required)

Join a financially sound, high-performing Federally Qualified Health Center (FQHC) that has served Central New York for over 50 years. This role is unique: you will serve as the primary full-time provider at our rural Marathon location, with a physician onsite once a week for support. We are looking for a dedicated clinician ready to make a long-term commitment to a community where we provide 50% of the primary care.

The Role: Comprehensive Rural Primary Care
  • Clinical Scope: Provide full-spectrum diagnostic and therapeutic care for patients of all ages, from infants to geriatrics.

  • Procedures: Perform minor surgeries (I&Ds, cyst excisions, laceration repairs), pelvic exams/paps, joint effusions, and initial EKG interpretations.

  • Caseload: Aim for approximately 3,200 visits annually, averaging 18 patients per day.

  • Team-Based Care: Collaborate within a "Dramatic Performance Improvement" model, working alongside care coordinators and nurses to support patient self-management.

  • Technology: Utilize Athena One EHR in a modern, well-maintained facility.

Schedule & Call Requirements
  • Work Week: 4-day clinical week (8:00 AM 5:00 PM) + 1 dedicated admin day.

  • Admin Flexibility: Admin time can be remote if paperwork and "desktop" tasks are up to date.

  • Call Rotation: After-hours call is required 3 4 times per month (including a weekend every 6 weeks).

  • Saturdays: Occasional Saturday shifts (8:00 AM 12:00 PM) paid at a per diem rate.

Submission Requirements
  • Education: Graduate of an accredited PA or NP program.

  • Licensure: Current New York State registration/licensure.

  • Certification: Board Certified or Board Eligible; current BLS/CPR.

  • Commitment: Ability to provide long-term stability to the Marathon community.
